The medical note must specifically state the class of service requested (business class or other higher-cost services) and the minimum number of flight hours to accommodate a medical condition.
- Please do not have the doctor state medical condition in the note. If the medical condition is included in the note, please redact such medical information before having your CAO/CFO co-sign the medical note.
- Your CAO/CFO must co-sign the Medical Note for Travel Exception prior to dropping the medical note in the box:
- The CAO/CFO will keep the original medical note on file
- All medical notes are valid one year from the date of issue
- Travel must be completed one year from the date of issue
