The Diabetes Center has arranged a program
through the UCLA Travel Center which allows Diabetes Center travelers to make their airlines
reservations and have the expense of the tickets charged directly to the
department. This will relieve travelers of the necessity of advancing
their personal funds and awaiting reimbursement at the conclusion of the trip.
In addition to this personal benefit, reserving through the UCLA
Travel Center will also provide the Diabetes Center travelers with access to the California
State Fares program. This program offers totally unrestricted fares
between many of the most frequently traveled "city pairs" at
contractually established rates. More information about the State Fares
program, including lists of instate & out-of-state city pairs can be found
on the UCLA Travel Center website under
Use of State Fares.
MAKING
RESERVATIONS
1) Complete the reservation request form (click on Reservation
Request in the QUICK LINKS, above) and submit it. The UCLA Travel Center
will contact you with a proposed itinerary and price.
When you have decided on
your final itinerary,
2) Obtain approval
from your Director by completing the Diabetes Center Authorization Request (select your
Director from the drop-down menu above in the QUICK LINKS) and submitting
it. The request will be emailed directly to your Director for approval.
When you receive
approval,
3) Contact your
travel agent and authorize ticketing.
